We know choosing a mental health provider can be difficult, and we want to thank you for considering us to be a part of your healthcare team. We are a holistic mental health and wellness private practice located in Tempe.  

 

For your convenience, we offer online scheduling for both in-person and virtual appointments. If you prefer in-office appointments, please note that our office is on the corner of Broadway and Mill on the 2nd floor of the Wells Fargo building. Once your appointment is confirmed, you will receive an email link to our Online Intake Form. The required Intake form must be completed within 24 hours of receipt. Your appointment will be cancelled if the Intake Form is not submitted within 24 hours so we can make the time available for other patients.  Please check your Spam and Promotions folder if you do not see the email or give us a call at 480-372-4135.

As a Psychiatric Mental Health Nurse Practitioner (PMHNP), I evaluate and diagnose a wide range of mental health conditions in people ages 13 to 64, prescribe medications and order diagnostics tests. I incorporate talk therapy in my follow up appointments, but I am not a therapist and therefore do not provide any extensive therapy. Although therapy is not a requirement of treatment, there is overwhelming evidence which tells us therapy with medication is the best clinical solution to most of the mental health issues we face. I encourage all my patients to find a therapist who is a “good fit” for them and provide referrals if needed.

If you are transferring care from another provider or requesting a 2nd opinion, you MUST complete a Release of Information

form. A link for this form can be located on our Home page. Transferring care does not guarantee a continuation of your current medication(s). This is at the discretion of the provider and will be determined at your initial appointment. If you have any questions regarding our prescribing practices, we would be happy to schedule a short call to answer any questions before scheduling.

 

As a solo practice, Spectrum Psychiatry DOES NOT have providers "on call" and as such, we only provide services during regular business hours. If you have an urgent matter that you feel cannot wait until the next business day, it is likely that your situation is emergent and would be better served to call emergency or crisis services.  

WE DO NOT OFFER EMERGENCY SERVICES.  However, existing patients may call during normal business hours to request an "urgent" appointment which will be evaluated by our staff and accommodated based on the nature of the situation and the availability of the provider. 

We do not do short term disability, long-term disability, or any court related documents. Medical documents/forms such as FMLA, ESA letter, school letters, Disability Resource Center forms or other Misc. Forms will be considered only for patients established for at least 2 months with few exceptions. A fee is associated with these services which insurance does not cover.

If you need to cancel or reschedule an appointment, you must provide no less than 24 hours notice. This will enable us to provide this time to another person waiting for an appointment.

A fee of $75 will be charged for late cancellations or missed appointments without notice given. Please call 480-372-4135 if you have any questions about our office policies.

All patients are required to keep a credit card on file in our secure EMR for self-pay, co-payments and in the event there is an outstanding balance or co-insurance due. All fees including self-pay and co-payments are pre paid and will be auto-charged to the credit card on file 24 business hours prior to your appointment. We accept all major credit cards.  If you are using insurance, your visit will be billed to your insurance company before any other balance is calculated. Depending on your specific insurance coverage,  you may end up with a balance due. If we are not in-network with your insurance plan, we can provide treatment to you out-of-network. You will be responsible for the cost of the services at the time of your appointment. We will provide you an invoice and you may be able to recoup some of the funds from your insurance company. However, this is not something we can guarantee

Although we assist in verifying your insurance, it is ultimately the client’s responsibility to verify insurance coverage including whether you have telehealth coverage.  Note: Some insurance carriers no longer accept telehealth services. We don't always know this until after we submit your bill.  Please verify your coverage and understand your coverage options for telehealth services prior to scheduling.
