Although we assist in verifying your insurance, it is ultimately the client’s responsibility to verify insurance coverage including whether you have telehealth coverage. Some insurance carriers no longer accept telehealth services. We don't always know
this until after we submit your bill. We recommend that you check with your insurance company prior to scheduling to verify your coverage including any specifics regarding mental health care. Please remember that verification of benefits is not a guarantee of payment, and you are ultimately responsible for payment of all services that are not covered by your insurance plan once the claim has been processed.

Please upload your insurance card and a photo ID when completing the new patient intake forms. It is very important to
inform us if there has been any changes in your coverage or in your address and phone number.
Changes may occur in the plans that we accept; therefore, we recommend confirming our acceptance at the time you make
an appointment.
We make every attempt to accept a wide range of insurance plans. However, all policies have different benefits, and we
cannot know the specific details of each individual policy. It is your responsibility to know your individual policy and to verify
all benefits and coverage information prior to having any services rendered. Also, you must notify us of any changes to your insurance plan or policy prior to your visit.

Co-Pays and Deductibles
Contracting with health insurance companies requires us to collect co-pays and deductibles. For patients with co-insurance and/or deductibles, we will be asking for a good-faith payment. A good faith payment is an estimate of what you will owe.
Once the insurance carrier adjudicates the claim, we may have to bill you for any remaining balance.
All patients are required to keep a credit card on file in our secure EMR.
All patient cost shares (co-payments, co-insurance, and deductibles) are auto charged to the credit card on file the morning of service.
If we are not in-network with your insurance plan, we can provide treatment to you out-of-network. You will be responsible for the cost of the services at the time of your appointment. We will provide you an invoice and you may be able to recoup some
of the funds from your insurance company. However, this is not something we can guarantee

If you do not have insurance or do not wish to use your insurance (self-pay), payment in full is auto-charged to the credit card on file 24 business hours prior to your appointment.
